Disability Support Worker – CARE @Metamorphosis Coaching

Are you passionate about making a meaningful difference in the lives of others? At CARE @Metamorphosis Coaching, we’re on the lookout for dedicated, values driven Support Workers to join our team and provide empowering, person-centred support to individuals living with disabilities.

This role involves working directly with participants in their homes and local communities, helping them build independence, achieve their goals, and feel truly supported every step of the way.

Your Role

As a Support Worker, you’ll be a vital part of someone’s journey. Your day to day responsibilities may include:

  • Assisting with personal care routines, daily household tasks, and meal preparation
  • Providing reliable transport and accompanying participants to appointments, community events, or social outings
  • Supporting clients to access education, employment, and recreational opportunities
  • Encouraging active participation in meaningful, goal-aligned activities
  • Administering medication or assisting with health-related tasks in line with their individual support plans
  • Creating a safe, inclusive, and respectful environment where each person feels heard and valued
  • Maintaining clear communication with participants, families, and other team members
  • Keeping accurate, timely records of supports provided, progress made, and any challenges or incidents
